• 2019 Teaching Artist Questionnaire -All roster artists interested in residency work (FY20) please complete for availability
• Artists in Schools Residency Checklist 
• AIS Contract template – completed contract will be sent to you for signature upon approval of application. Must be signed prior to residency and submitted to Art Education Director.
• W-9 form – must be completed prior to residency.
• Teaching Artist Survey – to be completed no later then 2 weeks after residency
